Job Summary

St. Thomas Community Health Center is looking to hire a non medical case manager.  The Non-Medical Case Management (NMCM) provides advice and assistance to empower clients to access a variety of necessary services to support medically related outcome measurements. Clients will have varying levels of needs throughout their enrollment in services. These services may include: medical, social, community, legal and financial. Clients who demonstrate a low level of need may benefit from NMCM. Enrollment in NMCM is not always permanent; as a client may move from one type of case management to the other depending on current situations. Frequent and ongoing assessments by case management (non-medical and medical) should occur to ensure clients receive the appropriate level of care.

Responsibilities

  • Completion of comprehensive assessment of service needs conducted by a non-medical case manager meeting the same qualifications as a medical case manager
  • Coordination of services required to support health outcomes
  • Periodic re-evaluation of client needs (housing, transportation, food, legal, emergency financial assistance, and other needed services).)
  • Encounter clients in their environment, which may include a residence, a public facility, in the streets, or in the facilities of the non-medical case manager
  • Inform the client of all Ryan White Part A funded services available, AND
  • Inform the client of all Ryan White Part A funded case management agencies in New Orleans Eligible Metropolitan Area (NO EMA), AND
  • What client can expect if s/he enrolls in case management services
  • Establish and/or verify client eligibility for services
  • Collect required state/federal client data for reporting purposes; AND
  • Completion of a full Client Eligibility Review and Certification Form (CERV)
  • Complete all Ryan White forms as required by HIPPA, Grantee, and the agency.
  • Complete and develop Ryan White Plan of Care in collaboration with the client in according to the client’s needs and follow-up on outcomes
  • Complete referrals for Ryan White services after assessing client
  • Upload eligibility documents into electronic records system within 3-days of eligibility
  • Answer and return phone calls, emails, faxes, and makes copies in a timely manner
  • Complete all Ryan White billing forms for every client interviewed, time clock corrections, and mileage on a weekly basis and submit to the Finance Department.
  • Work in collaboration with the medical case manager and medical case manager supervisor
  • Ensure recording of health indicators (CD4, viral load, acuity)
  • Monitor service utilization (including medical appointment adherence)
  • Treatment adherence counseling to ensure readiness for and adherence to complex HIV treatments
  • Establish whether client demonstrates a need or continued need to enroll in non-medical case management services offered by the service provider
  • Gather an on-going basis appropriate client information in order to determine client eligibility
  • Reassess at least every six month or as required to respond to a change in client status, utilizing assessment tools as mandated by OHP
  • Adhere to standards of service as outlined in the Non-Medical Case Management by the OHP

 Required Qualifications

  • High school/GED diploma required
  • Ability to communicate effectively with clients, families, physicians, etc., and fulfills documentation requirements.


Preferred Qualifications

  • Experience in community health or official public health agency.
  • Knowledge of HIV disease/AIDS.
  • Knowledge of case management.
  •  Bilingual in Spanish is preferred
